Information Sharing – How information about you helps us to provide better care

Information about you and the care you receive is often shared, in a secure system, by healthcare staff to support your treatment and care, The information is used to understand more about the health and needs of the General Public, to help make decisions about how best to use NHS funds and make sure that resources are used fairly and where they are most needed.

Information such as your NHS Number and postcode, but not your name, will be used to link your record to a secure system, so your identity is protected.  How your information is used and shared is controlled by law and strict rules are in place to protect your identity.

Please read the information below to help you decide if you wish to be part of the information sharing programme. If you do not wish for your data to be used in this way and wish to opt out please contact the surgery or complete the Opt Out Form at the bottom of this page and send this in to us.
